>>> politico is reporting that a framework for fiscal negotiations has already emerged. here is what they say to expect. about $1.2 trillion in tax increases on the wealthy. at least $400 billion in entitlement cuts mainly from medicare, not to mention spending cuts. that figure, at least $1.2 trillion, according to politico. let's bring in news nation political panel. nationally syndicated radio talk show host and msnbc contributor micha michael smir connish. zacha zachary carabel. zachary , as we look at this and what we know so far and the talking points that go back and forth, we haven't got specifics, details from the right. we hear what the left wants. isn't the onus on john boehner and the right to say exactly what they want.

>> i think it's on the onus of everybody to give a little in this. everyone talks about the simpson/bowles commission as a template for across the board reductions of long-term spending along with tax increases. that would obviously be ideal. right now we're talking about $2 trillion of spending decreases plus tax cuts , which even though running a trillion dollars of deficits a year, that's over a ten-year period. we're not talking a huge dent in the situation here. granted, any progress is probably better than no progress. but what we are talking about versus the ten-year horizon that we're facing, there is still a huge gap between washington in general and those economic realities.

>> the president has the leverage. the gop accusing the white house of failing to put forward the specifics. are democrats saying we told you what to expect farce revenues, now tell us what you want.

>> i think the democrats are really at this point just trying to play this game of chicken. i've just come from capitol hill just now. there was a press conference with harry reid and senator murray, senator schumer, senator durbin. i think what they're trying to do, this is a very high stakes game of chicken. they're trying to have the other side blink first and put out a detailed proposal they can then pick apart without having to do that first themselves.

>> meanwhile we have republican congressman tom cole , one of the many on the right out front saying he's willing to break the grover norquist tax pledge. basically urging the party to take the president's deal of extending the tax cuts for 98% of americans right away. i want to play what he had to say to our chuck todd earlier today, though.

>> the president says he wants to make 80% of the bush tax cuts permanent for 98% of the american people . and take them out of the discussion. i think we should do that. now, that advise was given in private.

>> you didn't intend for this to go public?

>> no, i absolutely didn't. again, it is what i said. it's not an inaccurate report.

>> all right. he didn't really want it to get out there, but it's out there. it's feathers to the wind. michael , you can't get this back. you know that well with radio. once you say it, it's out there. feathers to the wind. do you agree congressman cole is actually giving the speaker more flexibility in the negotiations?

>> yes. thomas, there's a tendency to watch these negotiations from the sidelines and to think that the prince iples speak for everyone. i think we learned two summers ago speaker boehner may be in a position where he's ready to cut a deal with the president but he can't control his caucus. to me the significance of congressman cole saying that is that it showed there's residual interest on the part of the constituency boehner must control to make this deal. you're right. you can't take it back. it seems quite obvious now those tax cuts are going to be extended for all but those earn earning in excess of $250,000.

>> the president has the leverage going into this, basically the mandate of a federal election and the fact that these, you know, the taxes are going to go up on everybody after the first of the year anyway. the republicans need to get something done now while they still have a little bit of wiggle room here. democrats need 25 republicans. that's it. to get a middle class tax cut through the house before the deadline on this, michael . then we've got -- there's cole . then there's tim scott from south carolina who's shown that he's flexible as well. are these people going to be critical, the critical congress people in getting a bill through the house?

>> they are. but i read the mandate a little differently than i think your question suggests. i think that the mandate was for compromise. i think that one of the takeaways, one of the nets of this election is that people liked seeing the president working with the man who delivered the keynote address for the republican convention , governor christie, together on superstorm sandy. that's what they want more of. i think members of congress --

>> there's no chris christie in washington . michael , there's no chris christie that's circling around washington , d.c., right now that actually needs something from the president. christie needed stuff from the president and fema and wanted it directly to get to his people. of course they're going to play nice .

>> i would like to think, though, the president was elected with a mandate. i can't say that. he doesn't control the house. he didn't have the house before. he doesn't have the house now. so compromise has got to be the word of the day . i think that that's what the constituents of these members most want.

>> all right. zachary , senator durbin says he could see $400 billion in entitlement cuts. democratic aides telling politico most of it is going to come from medicare in part by raising the retirement age. a move that more than two-thirds of americans oppose. that's according to the latest " washington post " poll. this is something that democrats are going to have a hard time signing off on.

>> remember, the deal that was at least we know tentatively struck in the summer of 2011 was a much more extensive one in terms of entitlement cuts. i think it was up to $2 trillion. that was going to be really difficult for the democrats then. now you're talking $400 billion over ten years. $40 billion a year. these things have to happen. the question is will they happen in the next two weeks. this is w simply are in a weak position . if we go to january 1st and the dynamic is everything is going up because the republicans will not agree to taxes on the rich, i cannot see how that is a politically winning hand or an economically winning hand.

>> we don't know all the things that are going on behind the scenes with these meetings with tim geithner today. but after the meeting with geithner senator mitch mcconnell released a statement accusing democrats of taking a step back, away from the consensus needed to bring compromise, get us away from the fiscal cliff. but i want to play what he had to say on the senate floor earlier today as well. take a listen.

>> the only reason democrats are insisting on raising rates is because raising rates on the so-called rich is the holy grail of libberloo. their aim isn't job creation . they're interested in wealth destruction.

>> wealth destruction, nancy. back to this wealth distribution thing and their whole narrative of punishing success. what role is mitch mcconnell going to serve in these negotiations at all?

>> he'll play a very key role. although you really have to keep in mind the final deal is really going to be struck between president obama and speaker boehner . and those are really the two people to watch. and their staffs right now. of course, mitch mcconnell will play a key role in bringing around senate republicans, excuse me. but it's really between the house republicans and the white house at this point. you know, we have to watch and see what they do. i think there's a lot of public posturing. but i do think that all these people have been at this table before. they're very familiar with the contours of what they will agree to from the debt ceiling negotiations. and so it's really just a matter of getting closer and closer to that deadline, having the markets start to freak out and then realizing that they have to come up with something.

Senex confirms new oil field discovery at Spitfire on the western flank of the Cooper Basin

Senex Energy Limited (Senex) is pleased to advise that successful exploration on the western flank of the Cooper Basin has confirmed the presence of an oil field at Spitfire.

The Spitfire-2 deviated oil exploration well targeted the mid-Birkhead Formation. It is located 530 metres north west of the Spitfire-1 oil exploration well in PEL 104 and approximately two kilometres west of the Growler oil field in PRL 15 (Senex 60% and Operator; Beach Energy Limited 40% in both permits). It was designed as a follow-up well to the Spitfire-1 exploration well, which delivered good oil shows but was plugged and abandoned as uncommercial in February 2012.

Ensign Rig 48 commenced drilling on 17 November 2012 and reached total depth of 1,900 metres on 23 November 2012. Wireline logs confirmed the presence of a gross oil column of nearly seven metres (vertical thickness) in the Birkhead Formation over the interval 1,713.9 metres to 1,720.7 metres (vertical depth) with interpreted net oil pay of 6.5 metres (vertical thickness).

Scientists See Advances in Deep Learning, a Part of Artificial Intelligence

A voice recognition program translated a speech given by Richard F. Rashid, Microsoft?s top scientist, into Mandarin Chinese.

Using an artificial intelligence technique inspired by theories about how the brain recognizes patterns, technology companies are reporting startling gains in fields as diverse as computer vision, speech recognition and the identification of promising new molecules for designing drugs.

The advances have led to widespread enthusiasm among researchers who design software to perform human activities like seeing, listening and thinking. They offer the promise of machines that converse with humans and perform tasks like driving cars and working in factories, raising the specter of automated robots that could replace human workers.

The technology, called deep learning, has already been put to use in services like Apple?s Siri virtual personal assistant, which is based on Nuance Communications? speech recognition service, and in Google?s Street View, which uses machine vision to identify specific addresses.

But what is new in recent months is the growing speed and accuracy of deep-learning programs, often called artificial neural networks or just ?neural nets? for their resemblance to the neural connections in the brain.

?There has been a number of stunning new results with deep-learning methods,? said Yann LeCun, a computer scientist at New York University who did pioneering research in handwriting recognition at Bell Laboratories. ?The kind of jump we are seeing in the accuracy of these systems is very rare indeed.?

Artificial intelligence researchers are acutely aware of the dangers of being overly optimistic. Their field has long been plagued by outbursts of misplaced enthusiasm followed by equally striking declines.

In the 1960s, some computer scientists believed that a workable artificial intelligence system was just 10 years away. In the 1980s, a wave of commercial start-ups collapsed, leading to what some people called the ?A.I. winter.?

But recent achievements have impressed a wide spectrum of computer experts. In October, for example, a team of graduate students studying with the University of Toronto computer scientist Geoffrey E. Hinton won the top prize in a contest sponsored by Merck to design software to help find molecules that might lead to new drugs.

From a data set describing the chemical structure of thousands of different molecules, they used deep-learning software to determine which molecule was most likely to be an effective drug agent.

The achievement was particularly impressive because the team decided to enter the contest at the last minute and designed its software with no specific knowledge about how the molecules bind to their targets. The students were also working with a relatively small set of data; neural nets typically perform well only with very large ones.

?This is a really breathtaking result because it is the first time that deep learning won, and more significantly it won on a data set that it wouldn?t have been expected to win at,? said Anthony Goldbloom, chief executive and founder of Kaggle, a company that organizes data science competitions, including the Merck contest.

Advances in pattern recognition hold implications not just for drug development but for an array of applications, including marketing and law enforcement. With greater accuracy, for example, marketers can comb large databases of consumer behavior to get more precise information on buying habits. And improvements in facial recognition are likely to make surveillance technology cheaper and more commonplace.

Artificial neural networks, an idea going back to the 1950s, seek to mimic the way the brain absorbs information and learns from it. In recent decades, Dr. Hinton, 64 (a great-great-grandson of the 19th-century mathematician George Boole, whose work in logic is the foundation for modern digital computers), has pioneered powerful new techniques for helping the artificial networks recognize patterns.

Modern artificial neural networks are composed of an array of software components, divided into inputs, hidden layers and outputs. The arrays can be ?trained? by repeated exposures to recognize patterns like images or sounds.

These techniques, aided by the growing speed and power of modern computers, have led to rapid improvements in speech recognition, drug discovery and computer vision.

Deep-learning systems have recently outperformed humans in certain limited recognition tests.

Last year, for example, a program created by scientists at the Swiss A.?I. Lab at the University of Lugano won a pattern recognition contest by outperforming both competing software systems and a human expert in identifying images in a database of German traffic signs.

The winning program accurately identified 99.46 percent of the images in a set of 50,000; the top score in a group of 32 human participants was 99.22 percent, and the average for the humans was 98.84 percent.

According to Aristotelian philosophy earth was eternal, a world without history and with no end. Only with the advent of religions based on the promise of a final salvation ? supposedly after the end of time ? it became of great interest for philosophers and scholars to calculate the age and understand the possible lifespan of earth.

Already in the 17th century astronomers and physicists, like Sir Isaac Newton (1642-1726), had gathered enough knowledge to exactly calculate and predict the motion of planets and stars. Taking the bible and other religious texts as a collection of true, but undated stories, naturalist used described astronomical events (like solar eclipses or comets) to calculate an absolute chronology for the bible. Various sacred chronologies were proposed, but most naturalists agreed that the time span in the bible comprised almost 6.000 years. As the bible begins with the creation of the world, also the age of the earth was set at 6.000 years.

However to reconcile the celestial chronology with events recorded in the layers of earth was much more difficult.

One of the first naturalists to introduce time into the geological record was the Danish anatomist Niels Stensen, or latinized Nicolaus Steno (1638-1686). Steno had studied outcrops of layered rocks in the landscape of Tuscany and developed a hypothesis to explain these layers. Every single layer was deposited during a time when the sea inundated the land, followed by a time when the sea disappeared and erosion took place.

Steno does not address the question when exactly these periods occurred, but he argues based on historic accounts and Etruscan ruins, still standing on top of the studied layers, that the area of Tuscany had not been inundated by the sea in the last 4.000 years. Supposedly the repeated floods occurred before this date and after the creation of the world.

One of the most promising works to match the bible with the geological record was published in the years 1680 to 1690 by reverend Thomas Burnet (1635-1715). Entitled ?The Sacred theory of the earth: containing an account of the original of the earth and of all the general changes which it hath already undergone, or is to undergo, till the consumation of all things? it describes how god created and also destroys earth using physical laws.

The history of the world is magnificently summarized in the frontispiece of the book. God is standing with his right foot on the primordial earth, creating a first, perfect garden Eden. During the subsequent deluge, the outer crust collapses, forming continents and mountains. For Burnet earth was a middle-aged planet and the freshness of the mountains and contours of the continents prove for a recent formation. The last pictures shows earth consumed by the apocalyptic fire and a new paradise established. Burnet?s work reflects the general knowledge at the time ? earth was relatively young and like the movement of stars its history organized in cycles.

The concept of cycles will prove essential in the further development of geology. Similar to Steno, also the Scottish naturalist James Hutton (1726-1797) uses the observable layers of earth to infer periods of deposition and erosion. However unlike Steno for Hutton, also based on his observations on the slow erosion of soil, these periods happened unimaginable long ago, as ?we find no vestige of a beginning, no prospect of an end?. Sir Charles Lyell (1797-1875) will adopt the idea of ?deep time? in his ?Principles of Geology? (1830), still imagining earth?s past organized in cycles.

?Principles of Geology? will profoundly influence a young geologist named Charles Darwin and in 1859 Darwin will publish a book, which will influence on its own the understanding of geologists of the history of earth.

Daily iPhone App: Lovely is real estate searching made easier - Tuaw

I've been apartment hunting lately (I want a little bigger place, with the option to finally get myself a dog), and here in LA, it's not an easy thing to do. Craigslist is usually the listings tool of choice, but it's so gummed with spam and vague listings here that it can be hard, especially when (like me) you're trying to be a little picky. And while there are a few subscription-based services around, I'm not really interested in paying a subscription to people who should be lusting after me and my perfect credit rating and dependable rent checks anyway.

I hadn't really considered using my iPhone in the search, but when a friend linked me an apartment listed on Lovely, I figured the app would probably be worth a shot. And I'm glad I gave it one: Lovely is, so far, the most useful free tool I've come across. There's a full web interface, but the app is also very simple and intuitive -- it's quick, responsive and very clear.

I particularly like the way pictures and maps are implemented; on Craigslist, the maps link is essentially an afterthought, but on Lovely, that information is put front and center, and loads up fast and clean, even on my iPhone's 4G while out and about. I am not quite sure how the Lovely listings are moderated (I know some of them do come from Craigslist, and you can filter in the app for listings with or without pictures, but it's not clear just which listings make it through or not, or exactly what all of their sources are). However it all works, though, I've found quite a few great places on the few days I've been watching the app, and I'm hoping that sooner or later here I'll find the place I'm looking for.

Perhaps I shouldn't recommend an app like this until I've actually found a place, but Lovely (available on the App Store now) is worth considering no matter what part of the relocation stage you happen to be in. I should also say that I'm looking in Los Angeles, where the community is relatively tech-savvy and the listings are plentiful, so if you're in a less populated or less tech-friendly area, Lovely might not work so well for you. And finally, there's no iPad interface, unfortunately, though I have to presume that's in the works. If you're setting out on an apartment search, or haven't found a good tool yet, give Lovely a shot for sure.

As the Mexican drug cartels obtain more control in Mexico, they are able to seep across the border and grab hold of the drug market in the United States. Cartels are able to easily transport their product across America, and to some this is proof the war on drugs is failing.

Piling it On

Insulation piled high in an attic

A comedy of errors can describe events that from a small misunderstanding snowball into a near catastrophe. Thankfully no gets maimed or killed, but some damage is done. Being a home inspector I view quite a bit of what I would call comedic measures that stem from a misunderstanding and a lack of general knowledge. Often the intention is noble, while the result is far from what had been intended.

Climbing through a scuttle hole to inspect an attic not long ago, I was immediately confronted with a large yellow mound. This hill it turned out was a long ridge of fiberglass insulation batts extending a good ways down the attic. Seeing an air handling unit and duct work, I suspected this heap of insulation was made while the HVAC equipment was installed and then never replaced. It was a good assumption as in my experience out of the way places like attics are often where some contractors leave messes.

I entered the attic and wended my way to the end and back. Along the way I saw several items of interest. The most apparent was the black stains on the roof sheathing. The stains were almost exclusively on the rear sheathing, the front was almost clean. This phenomenon is quite common to see in my experience and there is a very simple explanation.

Another item of interest was on some of the insulation that was still where it should be, undisturbed in the ceiling. The mound was created from a second layer which a past homeowner must have added to save energy.?The original insulation, which had at one time been covered, was spotted black. Again this a common sight to see with fiberglass insulation. The black stains are sometimes misidentified as mold when in fact they are nothing more than dirt.

This particular house had been previously inspected prior to sale. Based on that inspection repairs had been made. Some of those repairs were in the attic. I knew this by being shown receipts for the work performed.?The "repairs" made were two, one was a mold remediation. Apparently the inspector (I never saw the inspection report, all though it was offered. I opted to not view it) had recommend the "mold" on the roof sheathing be addressed. I saw the very detailed receipt for this work. The pictures in this post are the after. This cost the sellers about $1400.?

The front roof sheathing is quite cleanThe other "repair was for unblocking the soffit vents. Apparently the second layer of insulation had been cited as blocking air flow from the soffit vents. It must have been surmised this was the cause for the "mold". The repair according to another receipt was to remove the insulation from the soffit areas. So it turns out the heap of insulation was not entirely the result of the HVAC installation. The guy they hired to repair the blocked soffits had simply pulled the insulation away from the soffits and piled it up.

So at the end of day has anything been repaired?

NO!

There are several things happening in this attic that no one has correctly identified or as of yet repaired.

First the mold is certainly not a major concern, yes, it is a fungal growth, but it hardly warrants a $1400 dollar suspect remediation. I have said this before, the air in the attic does not, can not communicate with the living space. There are those who will debate otherwise, I'm not in the least convinced. The cause (not a symptom, i.e. the "mold") for the condition is due to several combine factors.

Black stains on the fiber glass insulation are from air by passesFirst the reason for the stains is moisture condensing on the roof sheathing and nails. The moisture comes from conditioned?air in the living space escaping?into the attic through bypasses and in this instance bathroom fans vented into the attic. The black stains on the insulation are due to air movement through these openings in the ceiling and the over all poor performance of the fiberglass insulation. Next the attic ventilation has been incorrectly installed and was also later blocked at the soffits. What no one apparently knew was the gable vents were causing a short circuit in the air flow from the soffit vents. With a ridge and soffit venting system, gable vents are not to be installed, period. This issue, as I said was not recognized.

The reason for black stains on the back of roof only is orientation of the house. The front of the house faces the sun. The sheathing at the front dries because of the sun beating on the roof. The solution for the attic is to first seal all by passes to prevent conditioned air from leaking into the attic. My advice would also be to get rid of the fiberglass batts, but that is not likely to happen. Next seal off the gable vents, install rafter vents and then replace the insulation. Done.

Sometimes the lack of good information and knowledge can pile on additional problems and costs.?

DC on pace for fewer than 100 homicides in 2012

WASHINGTON (AP) ? The crack epidemic that began in the 1980s ushered in a wave of bloodletting in the nation's capital and a death toll that ticked upward daily. Dead bodies, sometimes several a night, had homicide detectives hustling between crime scenes and earned Washington unwelcome monikers such as the nation's "murder capital." At the time, some feared the murder rate might ascend to more frightening heights.

But after approaching nearly 500 slayings a year in the early 1990s, the annual rate has gradually declined to the point that the city is now on the verge of a once-unthinkable milestone. The number of 2012 killings in the District of Columbia stands at 78 and is on pace to finish lower than 100 for the first time since 1963, police records show.

"It strikes me probably daily as I ride around the city, or sometimes when I'm sitting at home at night, and it's 10 o'clock and my phone's not ringing. Or I get up in the morning, and I go, 'Oh my gosh, I've slept five hours," said Police Chief Cathy Lanier, who joined the department amid violent 1991 street riots. "It strikes me quite often how different things are now."

The drop reflects a downward trend in violent crime nationwide and is in line with declining homicides in other big cities. Though killings have risen in Chicago, New York City officials say homicides dropped to 515 last year from more than 2,200 in 1990. Houston reported 198 homicides last year, down from 457 in 1985, while Los Angeles police reported fewer than 300 last year after ending 1992 with about 1,100. Across the country, violent crime reported by police to the FBI fell by 3.8 percent last year from 2010.

Though D.C. is hardly crime-free today, and crime in some categories is even up, the homicide decline is especially notable in a place where grisly acts of violence ? sometimes not far from the U.S. Capitol ? embodied the worst of the crack scourge.

The number of homicides in this city of about 600,000 residents averaged about 457 between 1989 and 1993, a staggering rate that attracted unwanted attention. "A war zone? No, Washington, D.C.," was the sub-headline of a 1992 People magazine story that described Washington as a "city under siege." The Economist in 1995 called it "the violence capital of America." Tony Patterson, a longtime homicide detective, recalled one eight-hour shift when every detective on his squad landed a homicide investigation. Drive-by shootings with multiple victims were common, as were witnesses who'd see something ? but say nothing.

The 1990 arrest of then-Mayor Marion Barry for smoking crack cocaine fed a perception that the city where the nation's laws were made was, itself, lawless.

"If you asked people what would happen first, there'll be a thousand murders in D.C. in a year or there'll be less than a hundred, I think virtually everybody would have said there would be 1,000," said John Roman, a senior fellow at the Washington-based Urban Institute.

Everyone agrees there's no single cause for the trend.

One overarching factor is the city's continued gentrification ? the 2011 median household income of $63,124 is higher than all but four states, census figures show. Whole city blocks have been refashioned, drug dens razed, a Major League Baseball stadium built in place of urban blight, high-rise public housing replaced by less-dense garden style apartments. Though the poverty rate has risen, the growing wealth has pushed impoverished communities farther away from the city center. Some crime has also migrated to neighboring Prince George's County, Md., though homicides are down there too.

"There are just more physical places in Washington, D.C., that are affluent and safe than there used to be," Roman said.

Law enforcement techniques and medical care have advanced at the same time. Improved technology helps officers pinpoint gunfire, even before a 911 call, and share information faster. A police unit dedicated to seizing illegal firearms was re-established and prosecutors, benefiting from the city's strict gun laws, routinely ask that defendants arrested on weapons charges be held without bond ? in part, to head off possible retaliation. Stronger community relationships mean detectives have developed better sources on the street and witness cooperation, police say.

And better medical care, honed through lessons learned in Iraq and Afghanistan, means patients who were once stabilized at the scene are more likely to be taken directly to the hospital, where they have access to improved blood transfusion processes.

"The advances in the way we practice nowadays, I think, probably helps today's trauma patient more so than 20 years ago," said Anthony Shiflett, an acute care trauma surgeon at MedStar Washington Hospital Center.

Still, homicides are but one gauge of a city's safety and an imperfect one too.

Crime in some other categories has risen this year from the previous year, including robberies and, as of mid-September, assaults with a dangerous weapon. There have been headline-making violent crimes in 2012, including the beating and robbery of a man returning home from a Washington Nationals game, the slaying of a taxi driver whose body was found inside his burning cab and, most recently, the fatal stabbing of an 18-year-old inside a subway station after a robbery.

U.S. Attorney Ron Machen, whose office prosecutes homicides, said that while witness cooperation has improved, retaliatory violence remains enough of a concern that he preaches against it during regular school visits.

"We always say, 'You're not going to remember what the argument was about five days from now, let alone five years from now. But you pick up that gun and shoot somebody, if you don't kill them, now they're going to be coming after you,'" he said.

The department doesn't track non-fatal shootings, but the number of aggravated assaults reported to the FBI ? which would encompass such crimes ? dropped from more than 8,500 in 1992 to 2,949 last year. The toll taken by non-fatal violence is apparent each Tuesday at the MedStar National Rehabilitation Hospital, where wheelchair-bound survivors gather for support sessions.

"I'm still frightened to a degree to come outside ? day, night, it really don't matter," said Jordon Cook, 31, who was hit by stray gunfire at age 15. "I'm probably going to have to deal with this until the day I die."

The homicide drop is good news for violence-weary residents such as John Harper, who said his street in the historically violent Anacostia neighborhood feels far safer than it did 10 years ago. Still, a fatal shooting last July on his block returned his thoughts to the night in 1999 when his own son was killed in an alley.

"I didn't even want to look over there because it just takes me right back to that day," he said, adding, "A lot of it is starting to come to an end, that behavior is starting to just leave this city ? hopefully for good. I know not all of it, but a lot of it."

Lanier, the police chief, said that in a city visited by millions of tourists annually, a continued downward trend might help alter a lingering perception of the city as a haven for violence.

"It really is about a vibrant, safe city. I want people to not only be safe but to feel safe," she said.

New test may improve cervical cancer detection

ScienceDaily (Nov. 23, 2012) ? Routine smear tests have considerably reduced the number of cases of cervical cancer, but despite intensive screening 250 women in Sweden still die from the disease every year. Researchers at Sahlgrenska Academy, University of Gothenburg, Sweden, have developed new methods of minimising the number of missed cases and making diagnosis more reliable.

Since the introduction of organised screening in Sweden in the 1960s, the number of women being diagnosed with and succumbing to cervical cancer has fallen dramatically. Screening, where a sample of cells is collected from the cervix and examined under an optical microscope, detects early cell changes so that they can be treated before they cause cancer.

However, despite intensive screening 250 women still die from cervical cancer each year in Sweden, and a further 500 develop the disease.

The sensitivity of the current test is low, which means that cell samples must be taken at least every three years. A large number of tests must also be repeated because of unreliable results -- something which causes anxiety among patients and additional costs for the health service.

Researchers at Sahlgrenska Academy, University of Gothenburg have now developed a complementary test capable of minimising the number of missed cancer cases.

"Around 70 per cent of all cervical cancer cases are caused by two specific virus types, known as HPV16 and HPV18. We have developed a method that identifies proteins of these oncogenic viruses in cells, enabling a more objective interpretation of the test results," explains Maria Lidqvist, a doctoral student, who presents the method in her thesis.

"This method can hopefully produce a more reliable diagnosis in uncertain cases and reduce the number of missed cancer cases, as well as the number of women who have to be re-called because of cell samples that are difficult to interpret."

The research behind this method has been financed by the Swedish Research Council and conducted at Sahlgrenska Academy, University of Gothenburg, in collaboration with Fujirebio Diagnostics AB in Gothenburg.

Relatives of young people who have died suddenly from a heart-related problem are at greatly increased risk of developing cardiovascular disease according to a study published online November 13 in the European Heart Journal [1].

The authors of the study say these findings have two major implications; firstly, they strongly support the fact that autopsies should always be performed in cases of young sudden unexplained death; and secondly, since the involved diseases are all treatable if identified in time, the findings suggest that close relatives of these victims should be screened to detect those who would benefit from preventive treatment.

Researchers identified 470 victims of sudden cardiac death (SCD), aged between 1-35, in Denmark between 2000-2006 and followed their first and second-degree relatives for up to 11 years. They found that relatives who were younger than 35 had a three-fold increased risk of any cardiovascular disease (CVD), a six-fold increased risk of ischaemic heart disease (reduced blood supply to the heart) and a more than 10-fold increased risk of cardiomyopathies (damaged or weakened heart muscle) and ventricular arrhythmias (potentially fatal disturbances in the rhythm of the heartbeat) when compared to the general population.

For young, first-degree relatives of SCD victims, the risks were even higher: a six-fold increase in ischaemic heart disease and a 20-fold increase in cardiomyopathy and ventricular arrhythmia.

Dr Mattis Flyvholm Ranthe (MD), a PhD student and research fellow at the Department of Epidemiology Research, Statens Serum Institut, Copenhagen, Denmark, who is the first author of the report published today, said: ?The most interesting findings were that risk of CVD overall was increased no matter what subgroups of SCD victims and relatives we examined, and that young (under 35 years) first-degree relatives overall had a four-fold increase in risk of any CVD, and nearly 20-fold increases in risk of cardiomyopathy and ventricular arrhythmia. Finding a risk factor associated with a four-fold or greater increase in CVD risk is rare!?

SCD is defined as sudden, unexpected death due to natural unknown or cardiac causes, with an acute change in cardiovascular status within one hour of death or, in unwitnessed cases, in a person last seen functioning normally within 24 hours of being found dead. In young people it is often caused by undiagnosed heart problems that may be hereditary, indicating that there may be genetic mutations involved in the condition.

Dr Ranthe said: ?If sudden cardiac death has genetic causes then this would suggest that relatives of young SCD victims are at greater risk of heart disease than the general population and would benefit from screening to identify those at risk so that they could be given appropriate preventive treatment. This could save a significant number of lives. However, as far as we are aware, there have been no population-based findings of this kind before, and current knowledge is limited and based on smaller descriptive studies.

?Our results show SCD, or the underlying heart problems, has a large hereditary component, and that relatives, particularly young, first-degree relatives, are at much greater risk of developing heart conditions compared with the general population. Family members of young SCD victims should be offered comprehensive and systematic screening, with the focus on the youngest and nearest relatives.

?Most of the cardiovascular diseases leading to sudden cardiac death are treatable, if diagnosed in time, before a fatal event. Our results reinforce the concept of screening as a rational ?tool? to identify such cardiovascular diseases in family members at risk, and thereby possibly prevent future sudden cardiac deaths. However, our findings do not suggest blanket screening of, for example, asymptomatic second-degree relatives of SCD victims, although cascade screening might trigger screening of more distant relatives after a CVD finding in a first-degree relative.?

An autopsy was performed in 67% (314) of the SCDs, and the autopsies confirmed a heart-related cause of death in 178 cases, with 136 remaining unexplained. Dr Ranthe said that conducting autopsies in such circumstances was important because having the cause of death correctly identified was vital information for the relatives, and also for guiding screening initiatives.

?Our study, along with findings from previous descriptive studies, suggests that screening in relatives of SCD victims should be driven by the suspected cause of death, based on autopsy findings and description of the circumstances leading up to the SCD. We would like to emphasise the importance of autopsy in guiding such screening initiatives; public awareness of the importance of autopsying all young SCD victims should be increased. Ideally, these autopsies would be carried out by pathologists trained in post-mortem cardiac examination.?

The findings on arrhythmia were also new and important, said Dr Ranthe. ?Previous studies of familial aspects of arrhythmia have primarily been centred around families with very severe arrhythmic syndromes. Our finding of dramatically increased general risks of ventricular arrhythmia is therefore novel and shows that the findings from heavily affected families are relevant on a broader scale and possibly just the tip of the iceberg.?

Future work by Dr Ranthe and his colleagues at the Statens Serum Institut will focus on examining familial clustering of cardiovascular disease in new large-scale register-based studies. Their co-authors at The Heart Centre, Unit for Inherited Cardiac Diseases, Copenhagen University Hospital (Rigshospitalet) are investigating SCDs in persons older than 35 years. They will be looking at survival and disease courses in the relatives of SCD victims and examining specific subtypes of SCD more closely; those remaining unexplained after autopsy are of particular interest.
